In 2000, mor= e than 10 million children died before they reached their fifth birthday. T= oday, it=E2=80=99s less than 5 million. The world cut child mortality in ha= lf, in just 25 years. It=E2=80=99s an amazing success story. But the story= =E2=80=99s not over yet. In fact, right now, the next chapter is being writ= ten as governments around the world set their budgets. And global leaders h= ave a once-in-a generation chance to do something extraordinary. The choice= s they make now=E2=80=94whether to go forward with proposed steep cuts to h= ealth aid, or to give the world=E2=80=99s children the chance they deserve = to live a healthy life=E2=80=94will determine what kind of future we leave = the next generation. To save as many children as possible, I'm urging leade= rs to increase health funding. But say they simply sustained current levels= . What would happen? Our foundation worked with the Institute for Health Me= trics and Evaluation at the University of Washington to find out=E2=80=94an= d the results were more hopeful than I expected. If the world invests in ch= ild health and scales lifesaving innovations, we could cut child deaths in = half again over the next 20 years. We know how to stretc= h every dollar to save the most lives. And the pipeline of affordable healt= h innovation is stronger than ever before. A suite of new approaches to mal= aria, including innovations that prevent mosquitoes from carrying parasites= , could all but eradicate the disease.=C2=A0New maternal vaccines can prote= ct babies from respiratory diseases, which are the biggest killers of newbo= rns. And long-acting HIV treatments and prevention options that replace dai= ly pills can drive AIDS deaths to single digits. With the right level of in= vestment and focus, it=E2=80=99s possible that HIV/AIDS, once the world=E2= =80=99s deadliest pandemic, could become a medical footnote. Countries arou= nd the world have the expertise to solve major health problems=E2=80=94but = they need the tools, medicines, and vaccines to make it happen. That=E2=80= =99s what global health institutions like The Global Fund and Gavi, the Vac= cine Alliance were designed to do: help countries make smarter and more cos= t-effective decisions. Where you live shouldn=E2=80=99t determine if you li= ve. I believe that opportunity shouldn=E2=80=99t be an accident of birth=E2= =80=94and where you live shouldn=E2=80=99t determine if you live. I=E2=80= =99ve announced that I=E2=80=99m giving away virtually all my wealth to sup= port this effort over the next 20 years, because this is a critical moment = for progress. But the fact remains: we won=E2=80=99t get there without rich= countries giving a small fraction of their budgets to help the world=E2=80= =99s poorest people and save millions of lives. That=E2=80=99s where govern= ment decisions come in. Of course, budget deliberations are difficult. Many= of the countries that have given the most health aid in the past face high= debt levels, aging populations, and domestic challenges. I=E2=80=99m not n= aive. I don=E2=80=99t expect most governments to suddenly restore foreign a= id to historic levels. But I am an optimist. Because while things may seem = dire right now, two things can be true at once: what=E2=80=99s happening to= the health of the world=E2=80=99s children is worse than most people reali= ze, and our long-term prospects are better than most people can imagine. That=E2=80=99s the paradox of this moment: fundin= g for global health is shrinking. But science is accelerating=E2=80=94and p= eople are determined to make sure innovations reach children who need them,= no matter where they happened to be born. The choices we make today will d= etermine what the world looks like for the next generation. I=E2=80=99m a g= randfather now, so this hits even closer to home. I want my grandkids to gr= ow up in a world where kids know what diseases like HIV, polio, and sickle = cell anemia were, but don=E2=80=99t know anyone who has them. A world where= malaria wards are empty=E2=80=94because no child is at risk of the disease= . A world where everyone survives childbirth=E2=80=94and childhood. That=E2= =80=99s the future every child deserves. And we can all play a part in buil= ding it. I=E2=80=99m talking to leaders about how to do more with less. Tha= t includes focusing on core lifesaving programs, doubling down on innovatio= ns that can eradicate and cure diseases, not just manage them, and laying a= pathway out of our outdated, donor-recipient model of global aid toward a = sustainable model where countries are self-sufficient. But none of that can= happen if donor countries don=E2=80=99t invest in the health of all people= , especially children. Take the Global Fund. It=E2=80=99s probably the most= effective lifesaving initiative of the 21st century: since 2002, it=E2=80= =99s saved 70 million lives from AIDS, tuberculosis, and malaria. Our found= ation will commit $912 million to the Global Fund over the next three years= . At the Global Fund replenishment in November, we=E2=80=99ll see just how = high of a priority this is for countries. Today, our foundation announced t= hat we will commit $912 million to the Global Fund over the next three year= s, and I=E2=80=99ll be interested to see what governments bring to the tabl= e. Low-income countries can also do more to prioritize the health of their = citizens. Many poorer countries spend less than 3% of their domestic budget= s on health. In many cases, it is not because they don=E2=80=99t want to sp= end more=E2=80=94it=E2=80=99s because they face crippling debt burdens and = interest repayments. Global financial institutions must relieve the burden = of debt so low-income countries can free up resources to invest more in the= health of their people. Yes, humanity is at a crossroads. But I=E2=80=99m = betting on humanity. Because no challenge is greater than the power of peop= le working together to solve it. Every day, I=E2=80=99m inspired by people = I meet, like Dr. Opeyemi Akinajo, who is working on AI-aided sensors that c= an help more moms and babies survive childbirth. Or Indonesia=E2=80=99s hea= lth minister, Budi Sadikin, who=E2=80=99s working tirelessly to ensure chil= dren have access to the health care they need to get the best possible star= t at life. I=E2=80=99m inspired by stories closer to home, too=E2=80=94like= Maddie and Emile Leeflang, teenagers from Utah who were moved by their vol= unteer work in Kenya to advocate for health aid. In every corner of the wor= ld, there are good people dedicated to making a difference. In every corner= of the world, there are good people dedicated to making a difference. Will= we be there for them? Will countries ensure their budgets support the heal= th of the world=E2=80=99s children? Will we ensure new breakthroughs reach = the people who need them most?
